Formula
VD and VT use the same volume unit · CO2 partial pressures use the same pressure unit · VD/VT is dimensionless
| Symbol | Meaning | Unit |
|---|---|---|
| Physiological dead-space fraction of tidal volume | fraction | |
| Alveolar CO2 partial pressure in the classical Bohr model | mmHg or kPa | |
| P̄_E,CO2 | Mean mixed-expired CO2 partial pressure in the same unit | same unit as Pa,CO2 |
\frac{V_D}{V_T}=\frac{P_{A,CO_2}-\bar P_{E,CO_2}}{P_{A,CO_2}}The classical Bohr equation uses alveolar PACO2 and mixed-expired P̄ECO2; their difference reflects dilution by CO2-poor dead-space gas. Clinical practice often substitutes arterial PaCO2 for PACO2: this is the Enghoff modification and can include ventilation–perfusion inequality and shunt effects. The equation assumes stable measurement conditions and representative mixed expired gas. A single end-tidal value is not automatically equivalent to mixed-expired gas.
PACO2 is alveolar and PECO2 represents mixed expired gas under stable measurement conditions.
CO2-poor dead-space gas dilutes expired gas; the relative pressure difference estimates the non-exchanging volume fraction.
Rearrange for dead-space volume
Classical Bohr form; Enghoff uses PaCO2.
PACO2=40, PECO2=28 mmHg, VT=500 mL. Find VD/VT and VD.
VD/VT=0.30 and VD=150 mL.
Silently substituting arterial PaCO2 into the classical Bohr equation.
PaCO2 identifies the Enghoff modification; classical Bohr uses alveolar pressure.
Formula cluster
a larger alveolar-to-expired CO2 difference indicates more physiological dead space.
With PACO2=40 mmHg and mixed-expired P̄ECO2=28 mmHg, VD/VT=(40−28)/40=0.30. If VT=500 mL, VD=150 mL. Check: mixed-expired CO2 is below alveolar pressure, so the fraction is positive. For Enghoff’s modification, PaCO2 is used in numerator and denominator instead.
Interpret physiological dead-space ventilation and ventilation–perfusion relations
Here is how to work through a typical Bohr Equation for Physiological Dead-Space Fraction (VD/VT = (PACO₂ − PECO₂)/PACO₂) task step by step:
Task
PACO2=40, PECO2=28 mmHg, VT=500 mL. Find VD/VT and VD.
Solution path
VD/VT=0.30 and VD=150 mL.
the tidal-volume fraction not effectively exchanging gas because of CO2-poor dead-space air. Its output is VD/VT as a fraction or VD as volume after multiplying by VT.. a larger alveolar-to-expired CO2 difference indicates more physiological dead space. The quantity answers a specific biological or statistical question, not automatically the full cause of an observed pattern. Tie it to the population, sample or model level from which inputs came. A number without that reference can be misleading across studies. Keep description separate from explanation: the metric summarizes data but does not by itself establish the underlying mechanism.
Use PACO2 is alveolar, PECO2 mixed-expired and VT tidal volume. The units are CO2 pressures need matching units; the pressure ratio is dimensionless.. Check that numerator and denominator refer to the same time interval, target population and scale. Proportions entered as percentages usually need conversion to values between zero and one. Write units and definitions beside the observations. This helps catch swapped groups, mismatched age intervals or a wrong denominator before they distort the result. Also document measurement definitions and preprocessing, because studies can operationalize the same symbols differently.
The central assumption is: mixed expired gas is collected representatively and alveolar gas is estimated suitably. steady breathing and no major short-term CO2-store changes apply. If Enghoff substitution of PaCO2 also reflects V/Q inequality and shunt., interpretation changes or the formula no longer fits. State sampling, measurement method and biological scale. A model is useful because it simplifies a question, but its limits belong in the report. Do not automatically transfer parameters to different species, populations, laboratories or sampling conditions. Report these assumptions with the result so readers can judge which populations and conditions the conclusion can cover.
In the example, PACO2=40 and PECO2=28 mmHg give a fraction 12/40=0.30. The check is: 0.30·500 mL gives a dead-space volume of 150 mL. Substitute calculated values back into the original relation and check whether ranges and units are biologically plausible. Rounded data can create a small discrepancy, but should not change direction or order of magnitude. Also test a simple limiting case, such as zero, complete detection or equal groups. This separates arithmetic errors from model limitations. Round intermediate values only at the end, and check the order of magnitude against the biological question.
A nearby quantity is the Enghoff-modified dead-space equation using arterial PaCO2. The substantive distinction is that Bohr isolates alveolar-to-mixed-expired dilution; Enghoff can include additional exchange impairment. Both can appear in the same biological analysis, but they use different inputs and answer different questions. Choose by the requested output and verify the data definition before comparing values. A related name does not imply identical units or interchangeable interpretation. This matters when a descriptive statistic is reported alongside a model parameter. Before comparing values, align definitions, populations, observation periods and analysis rules; otherwise the difference may be methodological.
